 * Headway is a platform that connects mental health providers with clients who have insurance. While I am not directly contracted with some of the insurance companies listed above, my credentialing through Headway allows me to provide services at in-network rates. This means you'll only need to pay your standard in-network copay, and I can offer services without the added expense of out-of-network rates.

If your insurance is not listed, you may still have out-of-network benefits.

To determine your coverage, consider asking your insurance provider the following questions:

• What is my out-of-network deductible? (This is the amount you must pay out of pocket before insurance starts covering a portion of the cost.)
• What is my coinsurance? (Unlike a copay, this is the percentage of the session cost you are responsible for after meeting your deductible.)

If you have out-of-network benefits, I will submit the required paperwork to assist you in obtaining reimbursement from your insurance company. I understand that out-of-network expenses can be a concern, so I aim to be flexible and accommodating. We can arrange to spread deductible payments over several months and may negotiate your out-of-pocket costs through a sliding scale fee based on financial hardship.

If you've had difficulty finding an in-network provider, you can contact your insurance company and request a single-case agreement. This allows them to cover my services at in-network rates for your specific situation.

Session Fees:

• $250 per 55-minute session
• Copayments/coinsurance are due at the end of each session.
